PHS makes it easy and convenient for patients to pay for their visits. We accept cash, checks, MasterCard and Visa as forms of payment. Additionally, we have a Charity Care program and offer prompt payment discounts for our self-pay patients. Please call our Billing Department at 740.420.8080 to find out more information.
All co-pays, patient cost share or self-pay balances are due at the time of service. Failure to bring your co-payment will result in your appointment being rescheduled.
We participate in a number of insurance plans. If we are not a participating provider with your specific plan, we will bill your insurance as a courtesy. Any balance after payment is received from the insurance plan is your responsibility and must be paid.
We have a $25 fee for each returned check. We charge a $30 fee for patients that do not show for their appointments and do not call to cancel.
