Patients Rights
It is the policy of PHS to respect and foster the rights and responsibilities of patients of all ages treated, to respect each patient's personal dignity and to provide considerate, respectful care focused toward the individual needs of each patient.
We are required by law to maintain the privacy of our patients' personal health information. At your first visit, you will be asked to sign a disclosure form, detailing to whom you wish your health information be released. Except for specific uses detailed in our in-office pamphlet, your information will not be disclosed or shared without your written information.
It is your responsibility to know and understand your insurance benefits. This will ensure you are not surprised by being financially liable for non-covered/non-authorized care.
